Partners in Health

New Hampshire Partners in Health (PIH) is a comprehensive community-based program designed to address the needs of children with chronic health conditions and their families. The program is currently available in 13 regions of the state through the efforts of Council for Children and Adolescents with Chronic Health Conditions and the Department of Health and Human Services.


Many families are faced with the challenges of raising children with chronic health conditions. At times, the issues may be overwhelming to all family members and the challenges can interfere with the quality of family life. Often, turning to family and friends is not enough...that’s when we need a friendly hand! This is why Partners in Health exists! Families deserve understanding, respect, support, and accommodations that address their needs within their homes and communities.


Eligibility Criteria for Enrollment:

·    Birth to 21

·    Documented chronic illness with verification from MD, DO, or RNP

·    The illness impacts activities of daily living

·    Family resides within New Hampshire
